Treatment Planning Sheet: used when making a complete diagnosis and setting treatment objectives. Your diagnosis should be global, capturing all major points of the patient's malocclusion. In limited orthodontics clinic, your objectives should be narrow, focusing in on only the small problems you will treat.

Informed Consent for Limited Orthodontics: every patient undergoing orthodontic treatment should have a *signed* consent form in the chart. It is your responsibility to review the potential problems of limited treatment with your patient. The patient should thoroughly understand that limited ortho is designed to fix only one or two aspects of their malocclusion and does not mean that comprehensive treatment wil be done.

Model Trimming: instructions on how to correctly trim models. Models should be poured in white stone and the mandibular tongue space blocked out with ALGINATE. Use the vacuum mixer to ensure a bubble-free mix. You must take a CO wax bite to properly trim. Damaged, stained, or poorly trimmed models will be rejected for diagnostic records.

Digital Photography: information on how to take extra-oral and intra-oral photographs

Photo Template: a simplified template in MS PowerPoint to assemble cropped photos. Specific instructions appear in the "notes" section on the slide. Be sure the turn on the "guides" under the "view" menu.

Bonding Technique: simple instructions on how to bond orthdontic appliances.

Interproximal Reduction Technique: instructions on how to do "IPR" or "slenderizing" to gain arch perimeter/length

Extrusion/Forced Eruption: instructions on how to orthodontically extrude a fractured or damaged tooth and improve ferrule by moving the tooth root out of the alveolar bone.

Debonding Technique: instructions on appliance removal

Retainer Info: information for patients on the wear and care of their retainers. Download and give to your patients.
